HavenPoint Assisted Living is committed to protecting the privacy of our residents' health information. This summary describes how protected health information (PHI) may be used and disclosed, and how residents can access that information, consistent with the Health Insurance Portability and Accountability Act (HIPAA).
We are required by law to maintain the privacy of PHI, to provide residents with notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.
HavenPoint uses administrative, physical, and technical safeguards to protect PHI, trains staff on privacy obligations, and limits access to those who need it to provide care.
This website is informational and is not a portal for health records. Please do not submit protected health information through website forms. Health information is handled directly through our care team under this notice.
