HavenPoint Assisted Living is committed to protecting the privacy of our residents' health information. This summary describes how protected health information (PHI) may be used and disclosed, and how residents can access that information, consistent with the Health Insurance Portability and Accountability Act (HIPAA).

Our commitment

We are required by law to maintain the privacy of PHI, to provide residents with notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.

How we may use & disclose PHI

  • Treatment — to coordinate and provide care with our staff and a resident's healthcare providers.
  • Payment — to arrange for payment of services where applicable.
  • Operations — for quality, training, and administrative functions.
  • As required by law — including public health, safety, and regulatory reporting.

Resident rights

  • Access and request copies of their health records
  • Request corrections to their information
  • Request an accounting of certain disclosures
  • Request restrictions on certain uses and disclosures
  • Receive a paper copy of the full Notice of Privacy Practices

Safeguards

HavenPoint uses administrative, physical, and technical safeguards to protect PHI, trains staff on privacy obligations, and limits access to those who need it to provide care.

To exercise your rights or file a privacy complaint, contact our Privacy Officer at hello@havenpointmn.com. You will not be retaliated against for filing a complaint. You may also complain to the U.S. Department of Health and Human Services, Office for Civil Rights.
